背景:癌症患者罹患失眠的比例約為30-60%,其中符合失眠症診斷的患者約有20%。失眠認知行為療法為治療失眠的黃金準則。然而,目前對於何種失眠認知行為療法傳遞方式可改善癌症病人失眠之成效仍未知。
目的:旨在以系統性回顧及網絡統合分析探討失眠認知行為療法之不同傳遞治療模式對於癌症患者的失眠治療成效。
方法:本研究為隨機臨床試驗之系統性回顧及網絡統合分析。主要納入失眠認知行為療法之不同傳遞模式比較其他非藥物性治療以改善癌症病人失眠之隨機臨床試驗。使用Cochrane library、Embase、PubMed、CINAHL等電子資料庫進行搜尋,納入之文獻不限年份及語言。經由兩位的研究者獨立透過The Cochrane collaboration’s tool for assessing risk of bias 2.0進行文獻品質評估。本研究採用隨機效應模型網絡統合分析方式進行統計分析,使用STATA 14軟體操作。
結果:共納入16篇隨機臨床試驗,平均年齡為56.6歲,癌症類別主要為乳癌。結果顯示,相較於常規照護組,個人失眠認知行為療法可顯著減少癌症病人之失眠嚴重度量表分數(-11.95, 95% confidence interval: -10.24 to -2.73)、增加總睡眠時間(139.67, 25.94 to 253.40)及提升睡眠效率(26.99, 5.56 to 48.41);團體失眠認知行為治療相較於常規照護組可顯著減少入睡潛伏期(-11.95, -20.97 to -2.94)及降低入睡後醒來時間(-19.83, -36.02,-3.64)。相較於常規照護組,自我引導失眠認知行為療法可顯著減少失眠嚴重度量表分數(-5.62分, -6.67 to -4.56)。個人失眠認知行為療法、團體失眠認知行為療法、自我引導失眠認知行為療法以及階段性照護失眠認知行為療法相比之下,對於治療變項皆無統計上之顯著差異。累積排行曲線下的面積結果顯示,針對失眠嚴重度量表及睡眠效率之改變,排序最佳之治療為個人失眠認知行為療法(89.8%與81%);主觀入睡潛伏期與入睡後清醒時間成效改變,排序最佳之治療為團體失眠認知行為療法(92.8%與90.3%)。
結論:個人及團體失眠認知行為療法之傳遞模式於癌症失眠上可能有較佳之療效。自我引導失眠認知行為療法可改善癌症失眠之嚴重度,可視為個人及團體治療的替代方案,解決CBT-I可近性以及供不應求的問題。

Background: The prevalence of insomnia in cancer survivors is estimated to be around 30-60%, with approximately 20% meeting the diagnostic criteria for insomnia disorder. Cognitive-behavioral therapy for insomnia (CBT-I) is considered the gold standard treatment for insomnia. However, the effectiveness of different delivery modes of CBT-I in improving insomnia in cancer survivors is still unknown.
Purpose: To systematically reviewed and conducted a network meta-analysis to investigate the treatment efficacy of different delivery modes of CBT-I for managing insomnia in cancer survivors.
Methods: This was a systematic review and network meta-analysis of randomized clinical trials. Only randomized clinical trials comparing different delivery modes of CBT-I with other non-pharmacological treatments to improve insomnia in cancer survivors were included. Electronic databases including Cochrane Library, Embase, PubMed, and CINAHL were searched, with no restrictions on publication year or language. Two independent reviewers assessed the quality of the included studies using The Cochrane Collaboration's tool for assessing risk of bias 2.0. The statistical analysis was conducted using the random effects model network meta-analysis method with STATA 14 software.
Results: Sixteen randomized clinical trials with an average age of 56.6 years were included, and the main cancer type was breast cancer. The results showed that compared to the usual care group, individual CBT-I significantly reduced the severity of insomnia as measured by Insomnia Severity Index (-11.95, 95% confidence interval: -10.24 to -2.73), increased total sleep time (139.67, 25.94 to 253.40), and improved sleep efficiency (26.99, 5.56 to 48.41). Group CBT-I significantly reduced sleep onset latency (-11.95, -20.97 to -2.94) and wake after sleep onset (-19.83, -36.02 to -3.64) compared to the usual care group. Self-helf CBT-I significantly reduced the severity of insomnia as measured by the insomnia severity scale (-5.62, -6.67 to -4.56) compared to the usual care group. There were no statistically significant differences in treatment outcomes among individual CBT-I, group CBT-I, self-help CBT-I, and stepped care CBT-I. The cumulative ranking curve analysis showed that individual CBT-I was the most effective treatment for changes in insomnia severity and sleep efficiency (89.8% and 81%, respectively), while group CBT-I was the most effective treatment for changes in subjective sleep onset latency and wake after sleep onset (92.8% and 90.3%, respectively).
Conclusion: Individual and group CBT-I delivery modes may have better efficacy for insomnia in cancer survivors. Self-help CBT-I can improve the severity of insomnia in cancer survivors and can be considered an alternative treatment option to address the proximity and supply-demand issues of CBT-I.
